Does Permanent Makeup Hurt? What Each Area Actually Feels Like
Permanent makeup is done with topical numbing, and most clients describe light scratching, vibration or pressure rather than pain. Brows are the least sensitive area; lips register tingling and warmth; the lash line is ticklish and makes the eyes water; scar tissue can feel almost nothing or more than usual. Areola work after a mastectomy is often felt less, because nerves were removed in surgery.
“Does it hurt?” is the first question at most consultations, and the truthful answer is not a number out of ten. It is a description, and it changes with the area. What follows is what clients actually report, zone by zone, plus the few things you can do beforehand that make a real difference.
What numbing does, and what it does not
Topical numbing agents are applied before pigment work for every service and stay in use through the session. What they remove is sharpness. What they do not remove is the awareness that something is happening: most people describe it as light scratching or slight pressure, with the vibration of the machine underneath it. If your reference point is a decorative tattoo done without numbing, this is a different experience.
Brows: the easy one
Brow work is usually the least sensitive of the facial areas, and noticeably so. Clients report light scratching, vibration and pressure, and most find the brow area considerably less reactive than lips or eyelids. An ombré powder brow appointment runs about 2.5 hours, and the sensation stays at that level throughout.
Lips: warmth and tingling
Lips are more alive to touch than brows, and it shows. The words clients use are tingling, warmth, vibration and a light scratching or pressure — not sharp pain. Numbing is applied once the lips have been mapped and remains in use for the whole session. Sensitivity is also uneven across the lip: some spots register more than others, which is normal and nothing to do with the work going wrong. A lip blushing appointment takes up to three hours.
One thing to raise in advance: a history of cold sores. Antiviral prophylaxis should be discussed with your physician before the appointment, and lips must be free of an active outbreak on the day.
Eyes: ticklish, and they water
The lash line is the area people worry about most and the one that surprises them least. The sensation is a ticklish vibration and pressure right along the lashes, and the area stays sensitive throughout — but the thing to expect is watering. Eyes water during liner work; it is expected and not a sign anything is wrong. Contact lenses come out before the procedure, and lash extensions are removed 48 hours beforehand.
Watering eyes during eyeliner work are the normal response of an eye to something happening near it, not a signal to stop.
Timing, for planning: lash line enhancement takes an hour and a half to two hours, winged eyeliner two to two and a half.
Areola and scars: the unpredictable ones
Areola restoration after a mastectomy is often felt less than people brace for, and for a plain anatomical reason: nerves are removed during the surgery, so many clients have no sensation in the area at all, and others keep it only in patches. Where sensation remains, it reads as pressure, tingling or light scratching, and the same lidocaine and numbing creams used for lip and brow work are used here.
Scar tissue is the genuinely unpredictable case. It can be almost without sensation or, the other way round, more sensitive than the skin around it — vibration, pressure, tingling and scratching are all possible on the same scar. Stretch mark camouflage sits in between: light to moderate scratching, vibration, tingling or pressure, with the intensity depending on where the marks are and how large the area is.
What makes any of it worse
A short list from the studio’s pre-care rules, all of which move the needle:
- No alcohol or caffeine for 24 hours before. Both raise sensitivity and bleeding.
- No blood-thinning medication or supplements for 48–72 hours, unless prescribed.
- No sunburn on the area — for brows, no sun exposure for 7–10 days before.
- Eat a light meal, arrive hydrated, and come with clean skin: no makeup, oils or sunscreen on the area.
Afterwards
The first days bring tenderness rather than pain: slight swelling of the eyelids after liner, swelling of the lips on days one and two, then dryness and flaking.
